What Is an Offshore Medical?
An offshore medical is a mandatory fitness-to-work assessment required for anyone working offshore in oil, gas, wind, or renewable-energy sectors.
The medical confirms that you are physically and mentally fit to work in demanding environments, ensuring safety for yourself and your team.
At Neptune Health, our OEUK / OGUK and RUK medicals are carried out by experienced occupational health clinicians who specialise in offshore, marine, and energy-sector fitness standards.
Updated to OEUK Medical Guidelines Issue 8 (October 2025)
We proudly conduct all offshore medicals in line with the Offshore Energies UK (OEUK) Medical Guidelines & Examiners Manual – Issue 8 (October 2025).
This latest update introduces new, evidence-based standards to improve consistency, worker wellbeing, and accessibility across the offshore sector.
What’s New in Issue 8 (2025)
OEUK Issue 8 introduced several important updates that our clinicians now fully apply:
Enhanced fitness-for-work criteria — updated thresholds for cardiovascular, respiratory, and musculoskeletal fitness.
Functional fitness testing — refined guidance for wind-turbine and confined-space workers.
Improved health-surveillance protocols for conditions such as diabetes, hypertension, and mental health.
Digital OEUK certification — electronic issue and verification of medicals.
Recognition of virtual assessments — allowing specific parts of the medical to be conducted remotely via secure video consultation.
By integrating these updates, Neptune Health ensures every certificate we issue meets the latest industry-approved requirements under OEUK Issue 8 (2025).

Chester Step Test
You can arrange a Chester Step Test alongside your OEUK (OGUK) or RUK Medical at Neptune Health in Birmingham. The Chester Step Test (CST) is a recognised aerobic fitness assessment used to evaluate cardiovascular health and physical fitness for offshore and wind-turbine workers. The test typically lasts around 10 minutes and involves stepping up and down on a calibrated platform while your heart rate and exertion levels are monitored. Our clinicians calculate your aerobic capacity and overall medical fitness, providing you with a Chester Step Test Certificate on the same day. You can book the Chester Step Test as a standalone assessment or easily add it to your OEUK / RUK medical appointment for a complete offshore fitness evaluation.
Bideltoid Measurements
As part of your offshore medical at Neptune Health Birmingham, we perform official shoulder width measurements required for offshore helicopter travel. This measurement determines your seating category for safe helicopter evacuation and must be uploaded to your Vantage card before travelling offshore. If your shoulder width exceeds 55.9 cm, you’ll be classified as ‘Extra Broad (XBR)’ under Offshore Energies UK (OEUK) guidelines. XBR passengers are required to: Be measured more frequently to maintain up-to-date certification Sit next to a compatible helicopter window seat that accommodates the measurement Upon completion, we provide a certificate of shoulder measurement, which can be added to your Vantage card profile immediately. You can book this shoulder measurement as part of your OEUK / RUK medical or as a stand-alone appointment at our SEQOHS-accredited Birmingham clinic.

This certification is a mandatory requirement for most offshore training centres and helicopter transit courses. Your assessment includes: A detailed health questionnaire focusing on lungs, ears, nose, and throat A chest and respiratory examination Review of any existing medical conditions that may affect safe breathing under pressure All Fit to Train medicals are conducted face-to-face by our SEQOHS-accredited clinicians, and certificates are issued on the same day. You can easily add this test to your OEUK / RUK medical or book it separately for an additional cost.
